Who pays Dish Network cancellation fee?

it may not be the cordial thing to do but personally I wouldn't pay- it would not affect your credit since it's in her name and the only thing she could do is take you to small claims court- and then the judge may or may NOT order you to pay- I wouldn't worry about it too much especially if you two aren't living together anymore- best wishes!
